Tend Complaints and Feedback Form

Use this form to report a problem, complaint, or safety concern about our software. We aim to acknowledge all submissions within 2 working days and provide a full response within 30 days, in line with our post-market surveillance obligations as a Class I registered medical device.

Important – patient safety

If you believe the software has contributed to patient harm or a near-miss incident, please also report this directly to the MHRA via Yellow Card (www.mhra.gov.uk/yellowcard) and inform your local clinical governance team.
